ReadMe: 

In addition to ReadMe file, there are total six documents for CKD phenotype: 

1. CKDalgorithm_V4.2.pdf is CKD algorithm defintion (main document). 

2. CKDalgorithm_V4.2_coding.txt lists all codes that used for phenotyping CKD (corresponding to Table 1 in CKDalgorithm_V4.2.pdf). 

3. CKD_dd_V4.xls is CKD co-variable data dictionary. It contains Demo_phenotyping_comorbidity, Cr_eGFR, urineProteinTest_UACR, urineProteinTest_UPCR, urineProteinTest_A24, urineProteinTest_P24, urineProteinTest_UAprotein, procedure, diagnosis Sheet. 

4. CKDalgorithm_V4_queryTemplate.sql is the paramaterized and modularized query template for assisting CKD portable implementation. We encourage you to use this query template for your implementation. Please contact the developers if you have any questions. 

5. CKDalgorithm_V4_validationInstruction.pdf and CKDalgorithm_V4_validationResults.docx are for validation sites for manual review instruction and review results recording.
