Gastroesophageal Reflux Disease (GERD) Phenotype Algorithm

Would it be possible to get definiton of which CPTs are procedures and which CPTs are surgeries? The CPT tab does not distinguish between the two, yet the types need to be sent in separate files.

Thanks Katie - good point. Realistically, there is no real need to require separate files - fine to send them combined.

I'm unclear about a couple of the hx_of_gerd_meds table:

1. drug_name: Is this the generic name of the drug?

2. drug_code: Is this the RxCui value? NDC?

I also noticed that in the hx_of_gerd table, there's a ADHD_ICD9 variable. Was this intended to be GERD_ICD9?

For the drug_name we prefer the generic name (as suggested in the comment) and for drug_code, the rxcui of the generic if available.

The ADHD_ICD9 variable is a typo that we corrected, but it looks like a version without the correction snuck into PheKB. Please ignore.
